A.   Notarial Services Requirements

 

    * Document must be notarized by the U.S. notary public

    * Signature and seal of the notary public must be certified/attested by the County Clerk or Secretary of State

    * Legalization Fee: US$ 20.00 per certification of the County Clerk or Secretary of State in cash or money order (payable to the Indonesian Consulate)

    * Cover letter from the company requesting the Indonesian Consulate General to legalize the document

    * By mail: self-addressed stamped envelope to return the document

    * This office requires a complete set of photocopies of the documents to be legalized for its file

    * Legalization hours: 10:00 am to 12:30 pm

    * Processing time: 24 hours

 

 B.   SURAT  KUASA

 

    * Telah melaporkan diri di KJRI New York.

    * Menyertakan photocopy paspor.

    * Document di ketik dengan rapi dan harus di tanda tangani di depan pegawai konsuler. Apabila melalui pos document harus ditandatangani dan disahkan didepan notary public.

    * Document harus mencantumkan alamat di Amerika dan No. Paspor.

    * Biaya $ 20.00 cash, apabila melalui pos pembayaran harus dengan money order, payable to : Indonesian Consulate; dan juga mengirimkan amplop kuning (brown) dengan membubuhi alamat Saudara yang disertai dengan perangko certified mail atau express mail untuk pengiriman kembali document tersebut.

    * Proses pembuatan : 24 jam.

C.   SURAT KETERANGAN LAHIR (Bagi anak-anak yang ayahnya Warga Negara Indonesia)

 

    * Bapak / Ibu  telah melaporkan diri pada KJRI New York, jika belum harus mendaftarkan diri terlebih dahulu di KJRI New York (lihat syarat Lapor Diri).

    * Menyertai photocopy paspor Bapak dan Ibu

    * Menyertai akte perkawinan (Marriage Certificate) orang tua.

    * Photo anak 2 lembar ukuran 2" x 2"

    * Biaya (Fee) $ 10.00 dan $ 15.00 (legalisasi akte kelahiran), pembayaran dengan cash atau money order payable to : Indonesian Consulate.

 

 D.   SURAT KETERANGAN NIKAH

 

    * Telah melaporkan diri pada KJRI New York, jika belum harus mendaftarkan diri terlebih dahulu di KJRI New York (lihat syarat Lapor Diri).

    * Akte Perkawinan (Marriage Certificate) yang dikeluarkan oleh Circuit Court for City of Country.

    * Photocopy paspor masing-masing suami dan isteri.

    * Biaya (Fee) US$ 20.00, pembayaran dengan cash atau money order payable to : Indonesian Consulate.

 

 E.    SURAT KEMATIAN (Bagi jenasah yang ingin dikirimkan dan dikebumikan di tanah air)

 

    * Surat Kematian (Death Certificate) asli dari Departent of Health and Human Resources.

    * Surat Keterangan dari Funeral Home, yang menerangkan bahwa jenasah almarhum / almarhumah (the deceased) bebas dari penyakit menular.

    * Surat Keterangan dari Funeral Home, yang menerangkan bahwa jenasah almarhum / almarhumah di formalin (embalm).

    * Paspor asli almarhum / almarhumah (the deceased).

    * Tidak dikenakan biaya (no fee).

What is meant by "Passenger's goods"?

1. Goods carried by passenger of ship / airplane in one trip.
2. Baggage arriving within 3 months after the passenger's arrival by
-- ship/airplane (un-accompanied baggage).

Passenger's goods can be categorized into:

1. Passenger's goods exempted from customs duties and other import duties and taxes.
2. Passenger's goods subject to customs duties and other import duties and taxes.

Passenger's goods exempted from customs duties and
other import duties and taxes:

1. Goods for personal use during trip such as clothes, shoes,
-- wrist-watch or bags at value not exceeding FOB US$ 250.00
2. Cigarettes not exceeding 200, or cigars up to 50, or tobacco
-- up to 100 grams and alcohol not exceeding 1 liter per adult.
3. Camera, video camera, radio/portable radio cassette, binoculars and
-- sport equipment in normal quantities required by foreign tourists
-- while in Indonesia.

Goods will be subject to customs duties and other import duties and
taxes if its value exceeds the fixed value, example:

1. Goods for passenger valued at FOB US$ 750.00, the customs duties
-- and other import duties and taxes will be applied to the balance
-- exceeding the fixed value, i.e. FOB US$ 500.00 (US$ 750.00 - US$ 250.00).
2. Goods per family valued at FOB US$ 1,500.00, the customs duties and other
-- import duties and taxes will be applied to the balance exceeding the fixed value,
-- i.e. FOB US$ 500.00 (US$ 1,500.00 - US$ 1, 000.00).

Important note:

1. Without the approval of the authorized institutions, the following items are
-- prohibited for importation to Indonesian Territory: narcotics, firearms,
-- gas-operated gun, ammunitions, spring gun, explosives, transceiver, cordless
-- telephone, colored photocopy machine, materials/pornographic publications,
-- printed materials in Chinese language/characters, Chinese medicines.
2. Importing of film, videotapes, video laser disc, long play should go through
-- Authorized Censor Institution.
3. Concealment / evasion of goods from inspection or giving false declaration
-- to the Customs and Excise officials are violations on which sanctions may be
-- imposed in accordance with the regulations in force.
4. For your nice trip, please declare your carried goods in a Customs Declaration
-- at arrival in Indonesia (only one Customs Declaration for each family).


GOODS, SUBSTANCE OR MATERIALS WHICH ARE PROHIBITED TO BE IMPORTED OR
EXPORTED IN THE TERRITORIAL OF INDONESIA WITHOUT PRIOR APPROVAL OF
THE AUTHORITIES

1. Narcotics, including:

1.1. Opium (Papa per Somniferum).
1.2. Coca (Erythroxylon) including the plant and its processed product.
1.3. Ganja (Cannabis sativa) including the plant and its processed product.
1.4. Crystals and its derivatives of Morphine and Cocaine.
1.5. Substitutes of narcotics (natural/synthetic).
1.6. Mixture of the above mentioned substances.

Import/export, possession, usage and distribution without the approval
of the respective authorities are strictly prohibited.

2. Explosive materials, including:

2.1. All types of ammunition, incendiary bombs, mines, hand-grenades.
2.2. All explosive goods.
2.3. Explosive materials used for other explosive goods.

Importing explosives for military purposes such as TNT, Nitro Glycerin,
and the approval of the Department of Defense and Security c.q. BAPENAB
Hankam should be obtained.

Importing explosives for industrial purposes such as Ammonium Nitrate,
Dynamite shall be carried out by PT DAHANA and it should obtain the
approval of the Police of the Republic of Indonesia.

3. Firearms and ammunition, including among other things:


3.1. Parts of firearms.
3.2. Cannons/flame-throwers and parts thereof.
3.3. Air gun, spring-gun, gas-operated gun.
3.4. Imitation of firearms, light-signal/alarm pistols, start pistols,
----anesthetic pistols and the like, including parts thereof.
3.5. All kinds or weapon filling (ammunition/bullets).
3.6. Bullet shells (mantel kogels).
3.7. Projectiles used for spreading dangerous gases.

Importing all types and sizes, the approval of the Police
of the Republic of Indonesia should be obtained.


4. Fireworks, including:


4.1. All types and sizes.
4.2. Happy crackers a kind of easily explosive fire-works).

Importing all kinds and sizes of fireworks is strictly prohibited.

5. Various kinds of books, printed materials, including:


5.1. Books, magazines and all types of paper printed materials,
---- in the Indonesian language/dialects.
5.2. Books, magazine leaflets, brochures, newspapers written in
---- Chinese letters / characters and in the Chinese language.
5.3. Paper printed materials for cigarette packaging and medicine
---- labels either in the Indonesian or in a foreign language.

Importing books and printed materials in the Chinese language for scientific purposes, the approval of the Office of the Attorney General and of the Department of Trade should be obtained.

Importing of books and printed materials specified under numbers 5.1 and 5.3
may be approved in the following cases:
a. Diplomatic Facility (Government Regulation, No. 8 Year 1957).
b. For educational/teaching purposes, including materials using Braille
-- letters (upon the recommendation of the Department of Education
-- and Culture).
c. Medicine labels and cigarette packages attached to such goods.

6. Gramophone-records, cassettes, videocassettes, films.


Importing of the above goods should through the inspection of the office
of the Attorney General and/or the Film Censor Board except in the
case of TVRI (Television of the Republic of Indonesia) for its broadcasting
requirements and the Diplomatic Corps and International Institutions.


7. Transceiver equipment, cordless telephone or telecommunication equipment.


Importing of the above equipment, the approval of the Department
of Tourism, Post and Telecommunication should be obtained.

8. Colour photocopy machine and parts thereof.

Importing the above, the approval of BOTASUPAL (Coordination Board
for Fight Against Money Forgery) should be obtained.


9. Certain plant species such as:


9.1. Quinine (Linchan Species).
9.2. Natural Orchids Dendrobium, Vanda and other species of Orchids.
9.3. Agave Sp (ecies).
9.4. Musa Textilles Mees.
9.5. Ranwoefia Sp(ecies).
9.6. Rafflesia Sp(ecies).

lmporting/exporting the above, the approval of the Department of
Agriculture should be obtained.

10. Plants and plant seedlings.

Importing the above, the approval of the Department of Agriculture
should be obtained and it should go through plant quarantine inspection.

11. Certain protected animal species, including parts or products thereof, among others:


11.1. Rhinoceros.
11.2. Orang-utan.
11.3. Bubulcus/Egretta.
11.4. Tapir.
11.5. Paradise Bird (Paradisea minor/ranggiana).
11.6. Buffalo.
11.7. Deer.
11.8. Chevrotin (Tragulus pygmaeus).
11.9. Mother of pearl, Troka, Roa and others.

Exporting the above, the approval of the Department of
Agriculture should be obtained.

12. Certain dangerous species of fishes, among others:


12.1. Piranha (Serrasalmus Sp).
12.2. Vampire Catfish (Vandelis Sp).
12.3. Alligator Gar ( Lepisostous Sp).
12.4. Silurus Slane.
12.5. Esex Masounjongy.
12.6. Electric Eel (Electrophorus Electicus).
12.7. Tetrodaoden Sp.

Importing of the above is prohibited except for special purposes
such as for zoological gardens or for scientific purposes, the approval
of the Department of Agriculture cq. Directorate General of Fisheries
should be obtained.

13. Several species of fishes, among others:


13.1. Sidat Breed (AnguillaSp)
----- below the diameter of 5 mm in size.
13.2. Fresh-water fancy fish of the Botia species,
----- above 15 cm in size (mother fish).
13.3. Fresh-water/river shrimps (udang galah),
----- below 8 cm in size.
13.4. Panacidae shrimps (Windu).
13.5. Bandeng (Chanos-chanos) Fish breed (Nener) in all sizes.

Exporting of the above is prohibited, except for scientific purposes
and for the promotion agricultural products, the approval of the
Department of Agriculture c.q. Directorate General of Fisheries
should be obtained.

14. Medicines, including:


14.1. Traditional ready-made Chinese medicines, in the form of pills,
----- capsules, powder, liquid and other ready to use forms.
14.2. Ready-made medicines produced abroad, including Chinese traditional
----- medicines "unregistered" at the Directorate General for Inspection of
----- Medicines and Food (POM).

Importing ready-made medicines or traditional medicines for personal use
or for trade, the approval of the Directorate General for the Inspection of
Medicines and Food should be obtained.

Ready-made medicines or traditional medicines may be imported in small
quantities for personal use by passengers of ships/aero planes as long as it
can be proven by a doctor's prescription.

15. Food and beverages unregistered at the Department of Health

Except in reasonable quantities carried by passengers of ships / aeroplanes
for use during the trip.

16. Solar Eclipse Glasses.

Importing of the above for observation purposes should obtain
the approval of the Minister of Health.

17. Dangerous Goods such as: easily explosive /flammable, combustible
--- chemicals, poisons, corrosives, oxidators, radioactive matters.


Importing the above, the approval of the Department of Health or,
in the instance of radioactive matters, the approval of the National
Atomic Energy Board (BATAN) and approval of the Department of
Trade should be obtained.

Importing radioactive matters as passengers' personal effects is prohibited.

18. Pesticides:


18.1. Pentachlorophenol and its salts.
18.2. Dichloride Diphenyl Trichloroethane (DDT).

Importing of the above, the approval of the Department of Agriculture
should be obtained.

19. Product of certain goods, among other things:

19.1. Rubber lumps, slabs scraps, unsmooked sheet, blanket sheet,
----- smooked sheet of certain quality.
19.2. Hides and raw (unprocessed) skin of cow, buffalo, goat,
----- sheep, horse, snake and others.
19.3. Rattan and rattan core.

Exporting of the above, the approval of the Department of Trade
should be obtained.

20. Waste, steel, copper and brass scraps.

lmporting and exporting of the above, the approval of the Department
of Trade should be obtained.

21. Goods of cultural value.

Exporting of the above should be approved by the Directorate General of
Culture cq. Department of Education and Culture and by the Department
of Trade.

W A R N I N G:
ACT (S) WITH THE INTENTION OF EVASION OR CONCEALMENT OF GOODS FROM BEING INSPECTED, OR GIVING A FALSE DECLARATION BEFORE CUSTOMS & EXCISE OFFICIALS IS A VIOLATION OF THE LAW, ON WHICH THE SANCTION (S) MAY BE IMPOSED.
